I will provide you with a “Super Bill” that contains the information insurance companies require to process a claim. You may submit this directly to your insurance provider. In some cases, the claim may be submitted on your behalf or as a courtesy. Any insurance reimbursement ​will be paid directly to you by your insurance company.
